Measles is not just a childhood rash. In Nepal, recent outbreaks have shown that measles returns quickly when vaccine coverage drops.
Short answer: Measles is highly contagious and can cause pneumonia, diarrhea, ear infection, encephalitis, malnutrition worsening, and death. Nepal’s 2023 outbreak involved hundreds of cases, mostly in under-immunized children, so MR vaccination and catch-up doses matter.
Why Measles Still Matters in Nepal
Many parents think measles is a normal childhood illness. That is dangerous. Measles can weaken immunity for weeks to months and can push an already undernourished child into severe illness.
For pediatricians, measles is also a signal. If we see measles, it usually means routine immunization has missed a group of children.
The Recent Nepal Outbreak
WHO reported that between 24 November 2022 and 10 March 2023, Nepal had 690 measles cases and one death. The outbreak was confirmed in Nepalgunj, Banke, on 2 January 2023.

The outbreak was controlled through case search, surveillance, outbreak response immunization, community engagement, and later a nationwide measles-rubella campaign in 2024 targeting about 5.7 million children.
Symptoms of Measles
- High fever.
- Cough, runny nose, and red eyes.
- Small white spots inside the mouth may appear early.
- Rash usually starts on the face and spreads downward.
- Child may look very tired, irritable, or unwell.
When Measles Becomes Dangerous
Go to hospital urgently if a child with suspected measles has:
- Fast breathing, chest indrawing, or low oxygen.
- Severe diarrhea or signs of dehydration.
- Drowsiness, seizure, severe headache, or confusion.
- Persistent high fever or inability to feed/drink.
- Age below 1 year, malnutrition, chronic illness, or immunocompromised state.
Vaccination and Catch-Up
Nepal gives measles-rubella vaccine through the national immunization schedule. If a child missed MR vaccine, parents should not assume it is too late. Ask the nearest health post or pediatrician about catch-up vaccination.
Because measles is so contagious, one missed child does not stay an individual problem. If many missed children live in the same area, an outbreak can start.
Isolation at Home
A child with suspected measles should be kept away from unvaccinated children, infants, pregnant women, and immunocompromised people. Inform the health facility before visiting if measles is suspected, so they can reduce waiting-room exposure.
My Take
Measles is one of the clearest tests of a vaccination system. If we want Nepal to eliminate measles, we need routine MR doses, catch-up for missed children, and quick action when fever-rash clusters appear.
